[caops-wg] Updated Authentication Profiles doc

CA-OPs,

I have made some changes to the profiles doc. The focus was changed a
little to be more generic. This is to allow none PKI solutions. I have
added a general outline for a profile. We still need Authors to write up
examples of existing operational profiles. Or we could cut that part of
the doc.
